The validity of Phoenix Card is for one year only from the date of activation

The card is to be used by the Card holder for getting free consultation services from the health service providers and other extra discounts and benefits as mentioned in the E-booklet.

4 . WHAT IS THE COST OF THIS CARD?

The cost of the card is Rs.500/- only and can be used for one individual person only.

5 . WHAT DOES ASP STANDS FOR?

ASP stands for Associate Service Provider who will be providing excellent healthcare service to the card holders.

6 . WHAT ALL BENEFITS CAN I GET FROM PHOENIX HEALTH CARD?

This card gives you not only free first consultation one time in a year but you can also avail discounts for radiology and pathology as many times as needed for the whole year.

7 . CAN I USE THIS CARD FOR ANY OF MY FAMILY MEMBER OR FRIENDS?

This is an individual health card and for one person only. Your friends and family members needs to buy individual card to avail its services.

8 . WILL I GET THE FIRST CONSULTATION OR OTHER SERVICES FREE OF COST?

Yes, your first consultation will be free by the service providers or as mentioned in the e-booklet but any other extra procedures if required will be chargeable as per the regular rates of the service provider.

9 . FOR WHAT SERVICES WILL I HAVE TO PAY EXTRA IN THE FIRST CONSULTATION?

Your first primary consultation if completely free . In case any further check up or procedures are required during first consultation , then they will be chargeable as per the regular rates of that particular healthcare provider .

10 . HOW DO I ACTIVATE THIS CARD?

11 . WHAT ADDITIONAL BENEFITS DO I GET FROM THIS CARD?

You will get additional 10% discount on the total indoor bills at most of the providers. The amount of discount provided is mentioned in the page of each particular healthcare provider.

12 . HOW DO I CONNECT THE HEALTH PROVIDERS MENTIONED IN THE E-BOOKLET?

Each healthcare provider has mentioned their contact details, the services they provide and facilities at their place in full detail. You have to directly call up the clinic of the healthcare provider to take an appointment.

13 . CAN I ORDER A HARD COPY OF THE E-BOOKLET? FROM WHERE?

There is an E-booklet which is present in the Phoenix Health Card which you have to download and keep it in your Smartphone. If there is still a requirement for a hard copy, you can call on the given helpline number and give your card details and address and we shall send a copy to use in 7 working days.

14 . WHAT DO I DO IF I LOSE THE CARD?

It is important for you to keep this card safely with you as it has huge benefits and is like a lifeline for you in case of any untoward circumstances . we request you to take a photograph of the card and save it your mobile as it will work just as well as your physical card .Still in the event of you losing the card, you can get a new card from the office address which will be provided to you in the next 7 working days after getting your request at a nominal charge of Rs. 100/-.

15 . WHAT DO I DO IF MY CARD GETS BROKEN OR DAMAGED?

The cards are made of a high quality plastic material which does not easily get damaged. If it does get broken or damaged, you may be denied treatment by the ASP and you will have to purchase a new card.

17 . WHAT IS THE LABORATORY DISCOUNT COUPON WHICH HAS COME ALONG WITH PHOENIX CARD??

The Laboratory free coupon ( voucher ) entitles you to two Rs. 250/- free blood reports you can utilize anytime in one year. The discount coupon is for SINGLE USE ONLY and has to be separately with a gap of few months . Any extra amount will have to be paid for the blood reports . For next time blood reports , you will get a 10% discount

18 . DO I HAVE TO CARRY ANY IDENTIFICATION CARD AT THE TIME OF THE VISIT ?

Yes, The ASP can ask for id proof and it has to be shown to them at the time of using the card.

19 . HOW CAN MY FRIENDS AND FAMILY MEMBERS CAN BUY THIS CARD?

The cards can be purchased from the mentioned outlet stores, the office address, online from the 40plus healthcare website, select mentioned chemist stores etc.

22 . CAN I USE THIS CARD IN EMERGENCY SITUATION?

This card is for consultation during the regular consulting hours only. If the provider has clearly mentioned in their information details, only then can it be used in emergency purposes.

23 . HOW MANY TIMES CAN I USE MY CARD AT PATHOLOGY LAB, RADIOLOGY LAB??WHAT DISCOUNT WILL I GET THERE??

You can use the card as many times as you want in the whole year at pathology and radiology centres. You will be provided a discount of 10% on the total bill for blood reports and 10% discount on total bill excluding consumables and medicines on radiology as many times as you need to use at mentioned centres.

24 . WILL I GET THE FREE CONSULTATION FROM ANY SERVICE PROVIDERS NOT MENTIONED IN THE E-BOOKLET??

No. This card entitles you to free first consultation from only those service providers mentioned in the E-booklet.

25 . WHERE DO I COMPLAIN IF THERE IS ANY PROBLEM REGARDING THE SERVICES RENDERED BY THE SERVICE PROVIDERS??

You can write your complaints on the website of the company and we will try to resolve in 3-4 days. Also you can call on the helpline number provided in the booklet for any queries if not solved in the FAQs.

26 . DO I GET ANY PRIORITY APPOINTMENT WITH THIS CARD??

No. This card entitles you to a free first consultation only. You will have to take regular appointment by calling the provider’s contact number and wait for your turn to be examined.

27 . HOW DO I RENEW MY SUBSCRIPTION AT THE END OF THE YEAR??

You can renew your subscription after the expiry of its validity by renewing it online on www.Phoenixhealthcard.com or buy a new card .

28 . WHY SHOULD I BUY THIS CARD WHEN I ALREADY HAVE A MEDICAL INSURANCE?

Mediclaim policy is mainly for indoor admissions and not for consultation. Also you have the benefit of availing discounts at laboratory and radiology centres for the whole year which insurance companies may not provide.

29 . CAN I USE MY CARD FOR ADDITIONAL TREATMENT ABOVE THE FIRST FREE CONSULTATION?

Yes. Where ever mentioned in the provider’s facilities and offers, you will get additional discounts on total indoor bills over and above the first free consultation.

30 . WHAT IS THE DISCOUNT PROVIDED FOR GYM MRMBERSHIP FEES?

We are pleased to inform that there will be a discount of 30% for women on Annual Gym Membership & 20% discount for men on Annual Gym Memberships . 10 % discount on personal trainings & 15% discount of Group Fitness Studio Membership for women only

32 . CAN I BUY THE HEALTH CARD ONLINE?

You can easily go on the website (www.Phoenixhealthcard.com) and buy new cards or renew.

33 . WHICH ALL SPECIALITY BRANCHES ARE COVERED BY THIS CARD?

There are more than 12 major specialities like gynaecium , dental , orthopaedics , eye , medicines , general and laparoscopic surgery etc and many more extended medical and paramedical services and supportive services which are all mentioned in great detail in the E-booklet .

34 . CAN I GET A REFUND IF I DON’T USE MY CARD?

No. you have one year to utilize the mentioned services.

35 . WHAT IS THE MINIMUM AGE LIMIT FOR USING THIS CARD ?

There is no minimum age. It can be used by all age groups.
